Community Care Connections offers a comprehensive outpatient treatment program for those struggling with substance use disorder. The program, led by board certified addiction specialists, including Director of Addiction and Recovery Dr. Benjamin Jackson, focuses on the individual needs of patients. Every recovery journey is unique... We meet patients where they are – in a judgement-free setting – and provide an individualized plan based on individual needs, delivered with compassion, understanding, honesty, and respect.

We help patients build a road map for long-term success and wellness. OUR PROGRAM
Our team, consisting of board-certified addiction specialists, case managers, peer recovery support specialists, and therapists, uses a variety of methods to treat addiction.
Medication Assisted Treatment
Medication Assisted Treatment (MAT) is an evidence-based program to treat substance use disorders. When appropriate, medications such as Suboxone and Vivitrol can assist with managing an addiction to alcohol, prescription drugs, or other illicit drugs. MAT works best when utilized within a full recovery program.
Case Management
Case managers support patients during recovery and ensure that comprehensive care is provided. They help create a treatment plan and guide patients to essential resources, such as program referrals, transportation, employment opportunities, and more.
Peer Recovery Support
Peer recovery support specialists are in recovery from substance use and use their personal experience to help others on their path to recovery. They walk alongside patients and help them meet the challenges faced during recovery; they can assist with goals, action plans, skills, and support throughout the recovery process.
Therapy
Individual and group therapy are crucial parts of the recovery journey. Therapy can help individuals get to the heart of their addiction, and it supports in the healing process from the disease of addiction.
HOLISTIC APPROACH
We care about patients' wholeness and complete healing. Our team supports and treats patients in multiple areas of life throughout their recovery journey and beyond, helping them find emotional, social, and physical healing from the disease of addiction.

ADDITIONAL INFORMATION
Community Care's addiction recovery program was launched n 2019 as part of our behavioral health department to address the unmet substance use disorder treatment needs of CCWV’s service area. Currently, we have addiction recovery programs in three locations: Community Care Connections of Bridgeport, Community Care Connections of Buckhannon, and Community Care Connections of Flatwoods. Our treatment team works with each patient to determine the level of care needed, and a master care plan is developed for a holistic approach to treatment, addressing chronic care, mental health, addiction treatment, recovery support, and social barriers.
The program accepts appointments and walk-ins and requires patients to adhere to program guidelines and attend therapy sessions.
PARTNERSHIPS
CCWV works with community partners through our Rural Communities Opiate Response Program (RCORP). This coalition consists of a multi sector group of organizations that work to address prevention, treatment, and long-term recovery. In addition, we are currently partnered with Upshur County Schools in providing services to their PALS program. The program aims to help students who have misused substances and allows them the opportunity to get ahead of the curve and start in early recovery. The program provides limited class sizes, as well as therapy opportunities. As the program develops, we look to expand into other schools and counties within our service area.
