For your convenience, we are excited to provide a Pre-Visit Check In process that can be completed on a smart phone, tablet, or computer. This service, called DocResponse, is secure and protects patients' privacy and protected health information. You will be able to verify and update your information, review and add any medical history, as well as complete scheduled assessments from the comfort of your own home. The link you receive will be tailored to you and the upcoming appointment. With the “paperwork” completed prior to your arrival, your CCWV Care Team can focus on addressing your healthcare needs.
If you have trouble with the check-in, we ask that you arrive 15 to 20 minutes early and bring a photo ID, insurance card, and co-pay. We will be happy to help you register for your appointment and answer any questions you have about DocResponse and Pre-Visit Check In.
