For more information, please talk with the pharmacist. The Connections program, through Community Care of West Virginia, is an Office-Based Medication Assisted Treatment (OBMAT) that is effective for treating patients with substance use disorders (SUD).
The program was established in 2019 and provides medication and therapy services, in order to help those individuals in recovery from substance use. The program works in conjunction with CCWV’s Behavioral Health program, as well as all CCWV locations. These services are provided in an out-patient clinic, which allows Community Care to expand access for addiction treatment, outside of the traditional medical settings. The program accepts appointments and walk-ins, and requires patients to adhere to program guidelines and attend therapy sessions. The Connections program currently has two Licensed Graduate Social Workers (LGSW) to provide therapy services to patients in the program. The program also features Case Managers, who work with patient’s needs and design a plan to help provide them with resources in the community, so that they can receive the best support and opportunities available to them. We also have on staff Peer Recovery Support Specialists (PRSS), who have the opportunity to share their stories with patients and assist them on a more personal level during their recovery. We are currently partnered with Upshur County Schools in providing services to their PALS program. The program is to help students who have misused substances, and allow them the opportunity to get ahead of the curve and start them in early recovery. The program provides limited class sizes, as well as therapy opportunities. As the program develops, we look to expand into other schools and counties within our service area. Our state-of-the-art Electronic Health Record (EHR) gives our Connections staff access to your records, while protecting that information from other Community Care departments. CCWV is now partnering with Health Note to provide pre-visit registration and texting services. Our previous texting number, 844-785-7409, will no longer be in service. You will receive a text from the new line before your next appointment. That text will include your link for registration paperwork and you will also be able to text your care team at that number for general questions or to make appointment changes.
