Be Involved With Your Health Care
As your Patient-Centered Medical Home (PCMH), you will work with a team of caring professionals and receive primary care that is focused on making sure your health care needs are met. As part of this care, we will coordinate all your care including office visits for routine and preventive care, referrals to specialists and follow-up on those visits, pharmacy needs and, when necessary, hospital care.

Patient Portal
