Be Involved With Your Health Care

As your Patient-Centered Medical Home (PCMH), you will work with a team of caring professionals and receive primary care that is focused on making sure your health care needs are met. As part of this care, we will coordinate all your care including office visits for routine and preventive care, referrals to specialists and follow-up on those visits, pharmacy needs and, when necessary, hospital care.

We look forward to working with you as a partner in managing your health care and encourage you to be involved with your health care. Participate actively with your Care Team to be the healthiest and happiest you can be. 

Patient Portal

Community Care patients have access to our Patient Portal 24 hours a day, seven days a week. Patients can see their medical records, send messages to their Care Team, schedule appointments, and pay bills using the Patient Portal. To register for or access our Patient Portal, click here.
