Our medical providers deliver compassionate, coordinated, high quality medical services to older adults, the frail elderly and disabled individuals. The providers can create a coordinated treatment plan that focuses on maximizing independence and functioning at home.
Enhanced pharmacy services are a key component of the In-Home Primary Care Program. Because many geriatric patients have multiple conditions, it is not uncommon for patients to take several different medications simultaneously. At CCWV, the medical provider can provide medication reconciliation which includes reviewing the accuracy and appropriateness of the medications, dosages and drug interactions.
Also, at CCWV, we have a pharmacist on staff who can consult with the medical provider and make suggestions to maximize medication effectiveness, cost and safety. By including the pharmacist in the care team, the health outcomes of the patient can be improved. These integrated pharmacy services are a critical part of how we deliver enhanced primary care to our community.
