Notice of Privacy Practices

Sliding Fee Program

No one will be denied essential medical services based on the ability to pay. If you are unable to pay for services, you may qualify for our Sliding Fee program. Discounts are available based on family size and income. For more information about the application and approval process, please see the front desk.

Mid-Level Providers

Mid-level providers make up all or a portion of the staff at this health center. If you have any questions, please ask the front desk staff or the Quality Services Coordinator or Patient Services Coordinator.

Notice Regarding Initial Payment

Please be advised, the amount you pay before seeing the provider may not be your final bill for the visit. You may receive an additional bill, dependent on the level of service and/or additional procedures performed by the provider.

Latex Allergies

Please notify the medical staff if you are allergic to LATEX GLOVES.

Medications on Site

There are NO NARCOTICS on these premises.

Service Animals

Trained and certified service animals are allowed inside the facility. No pets are allowed, for the protection of patients and staff.

Weapons

No weapons are allowed in this facility.

After Hours Call

Our patients can contact us after business hours at 1-888-557-2298. A provider is on call with electronic access to your medical information 24 hours a day.

Community Care offers extended hours on evenings and weekends. For specific clinic hours and information, visit a clinic location page.

Community Care locations observe the following holiday hours:

  • New Year's Day - Closed (CareXpress locations open 8 hours*)

  • Easter - Closed

  • Memorial Day - Closed (CareXpress locations open 8 hours)

  • Independence Day - Closed (CareXpress locations open 8 hours*)

  • Labor Day - Closed (CareXpress locations open 8 hours)

  • Thanksgiving - Closed

  • Day After Thanksgiving - Closed (CareXpress locations open regular hours)

  • Christmas Eve  - Closed (CareXpress locations open 4 hours)

  • Christmas Day - Closed

  • New Year's Eve - All locations open 8 hours*

*CareXpress locations are open normal hours of operation on holidays that fall on a Sunday, with the exception of Easter and Christmas.
