Complete and print the Enrollment/Consent form and email it to SBHCEnrollment@ccwv.org.

    • Well-Child exams

    • Chronic illnesses such as asthma, allergies, diabetes, obesity, sickle cell anemia and seizures

    • Accidents and injuries

    • Attention Deficit Disorder (diagnosis and treatment)

    • Nutrition counseling

    • School and sports physicals

    • Vision and Hearing screenings​

    • 24-hour on-call physician coverage

  • For urgent medical assistance after hours, please call 1-888-557-2298 to speak with a medical provider.

About SBHC

​A school-based health center (SBHC) is a doctor’s office inside the school. SBHCs provide comprehensive preventive and primary health care services and education to the students and faculty on a school campus.

CCWV is excited to be partnering with your child’s school. Our goals are to improve the health of the students, provide enhanced access to healthcare-related services that are not currently available, and supplement the services currently provided by the school nurses and counselors.
