SBHC Information for Parents

Community Care of West Virginia's School-Based Health centers are located in schools in Braxton, Clay, Harrison, Lewis, Pocahontas, Randolph, and Upshur counties. We look forward to working with you to help your children be the healthiest they can be.

Sports Physicals

Sports Physicals are available, free of charge, at all Community Care School-Based Health Centers and Community Care health center locations. Forms must be signed by a parent/guardian. Forms are available at all Community Care locations. If you would like to print a form in advance, click here.

Required Vaccines and Exams

West Virginia State Code and the West Virginia Department of Education require certain medical exams and vaccinations for enrollment in West Virginia public schools. Basic requirements are listed below.

  • The Department of Health and Human Services also has information available about childcare immunization requirements, Pre-K requirements, and college requirements.

Requirements by Grade Level

  • Oral Health Exam
    The Oral Health Exam documentation must be signed and dated by the student's dentist within the last 365 days prior to the first day of school. The forms for the exam are available in the dentist's office.

    Well Child Check-Up
    The Well Child Check-Up documentation must be signed and dated by the student's medical provider within the last 365 days prior to the first day of school. Forms are available from the medical provider. CCWV school-based health providers can perform these exams. Exams are also provided at any Community Care health center location.

    The Oral Health Exam documentation must be signed and dated by the student's dentist within the last 365 days prior to the first day of school. The forms for the exam are available in the dentist's office.

    Well Child Check-Up
    The Well Child Check-Up documentation must be signed and dated by the student's medical provider within the last 365 days prior to the first day of school. Forms are available from the medical provider. CCWV school-based health providers can perform these exams. Exams are also provided at any Community Care health center location.

  • Tdap & Meningitis Vaccines
    There are no provisional enrollments permitted for the 7th grade requirements. Students MUST show proof of these vaccinations to enter 7th grade. CCWV school-based health providers can administer these vaccines. Vaccines are also provided at any Community Care health center location.
