Job Objective:
The Care Manager is responsible to the Director of Nursing for rendering nursing care in assigned area in terms of individualized patient needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Director.

Position Responsibilities/ Essential Duties:
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient/significant other plan of care based on the identified patient problems/needs.
  • Carry out physician orders accurately, directly and indirectly.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient/ significant other identified problem/needs.
  • Document pertinent observation and intervention on appropriate record.

  • Perform limited I.V. Therapy procedures on those patients receiving peripherally inserted Intravenous fluids, as delegated by and observed by physician.  Verify the type of solution being administered. Evaluate the infusion site and the extremity for possible infiltration.
    Verify the intravenous according to the prescribed flow rate. Discontinue the intravenous at the appropriate time.
  • Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback.
  • Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care.
  • Effectively communicate patients changing needs and plan of care among staff members.
  • Report patient needs/problems and observations regarding patient’s condition to the charge nurse/physician.
  • Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
  • Provide patient/significant other health teaching as delegated by the Director of Nursing and/or the Charge Nurse.

Position Title: Patient Navigator (MA preferred)
Reports To: Regional Operations Manager
Position Type: Full-Time

Job Objective:
The Patient Navigator will act as an intermediary between the patient and the medical provider by facilitating regular behavioral risk assessments. In addition, the Patient Navigator will regularly interview patients in order to collect information related to social determinants of health (SDoH). The results of SDoH and the behavioral health risk assessments will be used to further medical and behavioral treatment, if necessary.
 
Responsibilities and Essential Duties:
  • Collect weight and height of patients and document in appropriate fields within the electronic health record and report collected data to the patient’s Care Manager for examination.   
  • Escort patient to exam room and initiate the patient check-in process. This includes verifying demographic information, emergency contacts, next of kin, preferred pharmacy, HIPAA Notice of Privacy Practices, income level, medical release forms, etc. and documenting in the correct fields within the patient’s electronic health record. 
  • Ensure that all signed paperwork is properly barcoded and then scanned into the patient’s electronic health record with the correct document name.
  • Register the patient for the patient portal, if applicable. Provide education as to the use of the system, highlight what communication can be made via the portal and the access the patient will have to their health information. Provide the patient with instructions on how to utilize the system and generate passcode that allows for initial access.
  • As applicable, review the CCWV New Patient Welcome Packet and have the patient sign the Receipt and Acknowledgement form.
    • Provide the patient with the following documents and review each:
      • Patient-Centered Medical Home Letter and Agreement
      • CCWV Mission Statement
      • Up to date list of CCWV medical facilities and pharmacies, addresses, and contact information
      • “After hours” telephone number
      • Emergency room utilization
  • Present the Screening, Brief Intervention, and Referral to Treatment (SBIRT) tool to qualifying patients and assist them in completing modules. Explain the purpose of the assessment, the importance of honest responses to questions, and how their responses will be kept confidential.
  • During the intake process, evaluate the responses to the social determinants of health (SDoH) questions. When responses indicate difficulty in access, conditions, availability to physical or social needs, provide a community resources packet to the patient. Report responses associated with “high risk” to the patient’s primary care provider immediately.
  • May be required to serve as medical receptionist depending upon need
  • Other duties may be assigned to meet clinic needs.
  
Qualifications/Requirements:
  • Possess strong organizational skills
  • Excellent verbal and written communications skills. Possess exceptional interpersonal communication skills.
  • Able to work collectively with the administrative team associates.

Position Title: Regional Nursing Coordinator  
Reports To: Director of Nursing

Department: Nursing/Administration

Job Objective:
Within the framework of the objectives and policies established by Community Care of WV, Inc, plan and coordinates nursing services with those of the medical and administrative systems. He/she participates as a member of the sites in the assigned region coordination team in planning, policy formulation, and decision making with particular reference to the role, functions, and operations of the sites in the assigned region’s nursing services. Relates effectively with other departments for continuity of care; maintains satisfactory relations with other departments and nursing unites; participates in all phases of education, maintenance of records, and upgrading of policies, procedures, and skill of personnel. Works closely with the Regional Operations Manager to ensure the work flow and productivity.

Responsibilities and Duties:
  • Responsible for managing, directing, and supervising nursing staff in the assigned region. Including the scheduling of staff based on need, managing call-offs, approving/declining absence requests, discipline and performing performance reviews including chart audits.
  • Defines and maintains the standards of nursing practice within the center.  
  • Assess the quality of care rendered.
  • Initiates the development of the policies and procedures that govern nursing services and other services under his/her position control.  Participates in planning and improvements. Implements new workflows.
  • Responsible for verifying employee credentials under his/her position control.
  • Responsible for staff performance, staff recruitment, staff retention, and staff development (educational training). Evaluates performance of personnel with recommendations for appropriate action.
  • Makes nursing assignments appropriate to skill level of employees. Is responsible for assigning nursing task and job duties to nursing staff and performing proper follow-up to determine if tasks were completed and performed correctly.
  • Responsible for collaborating with other department and agencies (BCCSP, VFC, Family Planning, etc.) regarding resources and services for improved patient care and staff development. Will provide oversight to LPN’s assigned to above programs and monitor for guaranteed accuracy and compliance.
  • Evaluates the quality and cost-effectiveness of staffing and services within the goals of CCWV.
  • Arranges to have needed supplies and equipment on hand and in proper working order. Performs regular inventory management and submits order to proper personnel.
  • Participates in appropriate meetings, safety trainings and committees.
  • Will serve as the trainer of new hires using the Electronic Health Record (EHR), giving instruction of all essential processes such as proper documentation and usage of the system.
  • Monitors weekly QM, MU reports and addresses areas of improvement with staff within assigned region. Performs proper follow through to assure improvements have been made and goals are attained.
  • Coordinates daily, weekly and monthly Huddle meetings with staff, regional operations manager(s), to review Athena updates, workflow configurations, quality outcomes, etc.
  • Generate quality reports for staff, providers, administration and review daily with appropriate staff.
  • Provides clinical expertise in the management of medical emergencies. Using resources appropriately and in a clam manner. May be called upon to administer medications and intravenous solutions skillfully and correctly.
  • Is prompt and efficient with minimal absences.

  • Will provide education training to staff on current and new workflows with continual evaluation on implemented processes and workflows.

Position Title: LPN -Pain Management

Job Objective:
Provides direct and indirect nurse driven care in the interventional pain management setting in both a clinic and procedures component under the supervision of designated pain management physician and the Site Coordinator.  Pain Management Nurse is motivated to provide patient care continuity that reflects initiative and responsibility indicative of professional expectations directed toward meeting both the psychological and physical needs of the patient in a productive, efficient manner. Pain Management Nurse has the ability and desire to be part of a high intense fast paced environment working in concert as a valued and integral member of a team in providing effective quality care. Pain Management Nurse communicates effectively on behalf of the patient and the physician reflective of Community Care of West Virginia Inc.’s mission and values as well as the scope of service of the department.

Responsibilities and Duties:
  • Prepares patient records and supporting documentation (Board of Pharmacy) for all scheduled patients into the clinic in a timely manner. Directs patient care movement from the waiting area, examination room, and to checkout.   
  • Provides nurse directed care prior to physicians clinic visit with patient including health systems review, recording vitals, medication reconciliation, addressing patient concerns pertinent to evaluation. Assists the physicians clinic visit as needed with direct patient care.
  • Follows up with the patient and enters any physician orders, provides any necessary educational information, and obtains necessary information or consent to provide recommended therapies.
  • Documents appropriately in the patient’s paper and/or electronic medical record all pertinent information in accordance with clinic wide policy and procedures.
  • Prepares all chart records and supporting documentation for all patients scheduled in a timely manner.
  • Supports the pain management physician as needed during procedures in accordance with nursing standards. Constantly monitors and is responsive to changes to the patient health status in accordance with nursing standards.
  • Manages pain management departmental supply inventory. Works with proper staff in submitting requests for additional supplies and ensuring all supplies are inventoried and in good working use.
  • Submits all paper chart records for scanning following clinic or procedures day.
  • Handles all patient care results and inquiries in a timely manner. Follows up with patients with any pertinent information directed towards their care such as appointments, test results, etc. in a timely manner.
  • Takes initiative and responsibility for meeting patient needs in a productive, efficient manner. Shows ability to perform in a high intense, fast paced environment.
  • Acts as a valued and integral member of a team in providing quality, effective care.
  • Maintains all regulatory requirements and adheres to all clinic policies and standards.

  3. Maintain accountability for medication inventory; immediately notify supervisor of discrepancies.
  4. Support the facility's medical supply system; follow procedures for patient charge items.
  5. Correlate accurate and complete documentation and its impact on reimbursement and federal regulations.

Other Duties:
Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances. May require travel (no overnight) to another CCWV health center. May also be required to attend job specific training sessions annually.

Education, Knowledge and Skills:
  • Graduate of an approved school of nursing; current West Virginia license to practice as a licensed practical nurse.

Job Objective:
The Licensed Practical Nurse (LPN) is responsible to the School Based Health Program Coordinator for rendering nursing care in assigned area in terms of individualized patient needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Coordinator.

Position Responsibilities/ Essential Duties:
  • Assist medical provider in implementing school based health center program and promote its purpose, function and success.
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient plan of care based on the identified patient problems/needs.
  • Carry out medical provider’s orders accurately, directly and indirectly.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient identified problem/needs.
  • Document pertinent observation and intervention on appropriate record.
  • Perform selected tasks related to patient care and the patient's environment in a safe manner.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes. Document all injections, medications and therapies administered in appropriate records. 
  • Maintain and coordinate communication among patient, staff, and medical provider as evidenced within medical record, client, and health care team feedback.
  • Assist in identifying and reporting suspected child abuse and neglect

Education/Requirements/Qualifications:
  • Valid license as an LPN in the State of West Virginia. 
  • Graduation of an accredited Licensed Practical Nursing Program.
  • Current Basic Life Support (BLS) certification.
  • One year experience working with pediatric and adolescent patient is preferred.

The Licensed Practical Nurse (LPN) is responsible to the School Based Health Program Coordinator for rendering nursing care in assigned area in terms of individualized patient needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Coordinator.

Position Responsibilities/ Essential Duties:
  • Assist medical provider in implementing school based health center program and promote its purpose, function and success.
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient plan of care based on the identified patient problems/needs.
  • Carry out medical provider’s orders accurately, directly and indirectly.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient identified problem/needs.
  • Document pertinent observation and intervention on appropriate record.
  • Perform selected tasks related to patient care and the patient's environment in a safe manner.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes. Document all injections, medications and therapies administered in appropriate records. 
  • Maintain and coordinate communication among patient, staff, and medical provider as evidenced within medical record, client, and health care team feedback.
  • Assist in identifying and reporting suspected child abuse and neglect
  • Other duties as assigned

Education/Requirements/Qualifications:
  • Valid license as an LPN in the State of West Virginia. 
  • Graduation of an accredited Licensed Practical Nursing Program.
  • Current Basic Life Support (BLS) certification.
  • One year experience working with pediatric and adolescent patient is preferred.

Job Objective:
The Care Manager is responsible to the Director of Nursing for rendering nursing care in assigned area in terms of individualized patient needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Director.

Position Responsibilities/ Essential Duties:
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient/significant other plan of care based on the identified patient problems/needs.
  • Carry out physician orders accurately, directly and indirectly.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient/ significant other identified problem/needs.
  • Document pertinent observation and intervention on appropriate record.
  • Perform selected tasks related to patient care and the patient's environment in a safe manner.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes
  • Perform limited I.V. Therapy procedures on those patients receiving peripherally inserted Intravenous fluids, as delegated by and observed by physician.  Verify the type of solution being administered. Evaluate the infusion site and the extremity for possible infiltration.
    Verify the intravenous according to the prescribed flow rate. Discontinue the intravenous at the appropriate time.
  • Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback.
  • Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care.
  • Effectively communicate patients changing needs and plan of care among staff members.
  • Report patient needs/problems and observations regarding patient’s condition to the charge nurse/physician.
  • Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
  • Provide patient/significant other health teaching as delegated by the Director of Nursing and/or the Charge Nurse.
  • Use available written materials specific to patient condition or disease.
  • Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion
  • Document health teaching including patient/significant other response to and their understanding of health teaching.
  • Participate in staff development programs and staff meetings.
  • Attend scheduled staff meetings and is responsible for information discussed.

Education:
  • High School Diploma or equivalent
 
Licensure/Certification:  
  • Valid registration as an LPN or RN in the State of West Virginia or Certified Medical Assistant
  • Current Basic Life Support (BLS) certification.
 
Experience:
  • One year experience in medical/surgical nursing and/or area of specialty preferred.

Position Title: LPN/Community Health Worker

Job Objective: 
The Community Health Worker (CHW) will be responsible for helping patients and their families navigate and access community services, other resources, and adopt health behaviors.   The CHW supports medical providers and the quality team through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family. Provide social supports and informal counseling, advocate for individuals and community health needs. Community outreach, such as home visits will be required.

Position Responsibilities/Essential Duties:
  • CHW will identify high-risk patients with conditions such as diabetes, COPD, etc. and provide home visits to improve health outcomes, reduce complications and to improve communication between patients and providers. Assist patients in self-management of chronic illnesses and medication adherence as directed by their medical provider.
  • Responsible for establishing trusting relationships with patients and their families while providing general office support and encouragement.
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with patients and families.
  • Follow-up with patients via phone calls, home visits and visits to other settings where patients can be found.
  • Help patients set personal goals, provide coaching and resources to help them achieve reasonable goals.
  • Provide referrals for services to community agencies as appropriate.
  • Help patients connect with transportation resources and give appointment reminders in special circumstances. CHW’s transporting patients is strictly prohibited.
  • Working closely with medical providers to help ensure that patients have comprehensive and coordinated care. Follow-up with patients should be continuous from initial identification through closure.
  • Be responsible for providing consistent communication to the Director of Quality Improvement to evaluate family/patient status, ensuring that provided information and reports adequately describe progress.
  • Be knowledgeable about community resources appropriate to the needs of families and patients.
  • Record patient care management information in the electronic health record in a timely fashion.
  • Other projects and duties as assigned.

Education :
  • High School Diploma
  • Graduate of an accredited Licensed Practical Nursing Program
 
Licensure/Certification:
