About West Milford

Community Care of West Milford offers comprehensive primary care and chronic disease management for people of all ages, as well as a pain management program. This location also offers convenient mail-order pharmacy services. We are currently accepting new patients. Call or schedule your appointment online.

  • The pain management program, licensed by the State of West Virginia’s Office of Health Facilities Licensure and Certification, addresses pain issues arising from both physical causes, like injury and illness, and mental causes, like mental illness and addiction.   • Community Care pharmacies participate in the 340B discount drug program, which provides discounted medications for qualifying patients. We also offer other convenient services, such as medication synchronization and mail-order pharmacy services.
