About Clay
Community Care of Clay offers comprehensive primary care and chronic disease management for people of all ages, as well as walk-in acute care for non-chronic, non-life-threatening illnesses and injuries. This location also offers pharmacy services through our in-house pharmacy. We are currently accepting new patients. Call or schedule your appointment online.

Community Care Connections, Addiction Medicine, offers a comprehensive outpatient treatment program for those struggling with substance use disorder. We provide an individualized plan based on individual needs, delivered with compassion, understanding, honesty, and respect.
