About Clay

Community Care of Clay offers comprehensive primary care and chronic disease management for people of all ages, as well as walk-in acute care for non-chronic, non-life-threatening illnesses and injuries. This location also offers pharmacy services through our in-house pharmacy. We are currently accepting new patients. Call or schedule your appointment online.

  • Clay is also a walk-in care location, open seven days a week, because we know that illnesses and injuries don’t occur at convenient times. No appointment is necessary… Just walk in when you need us, and you will be seen quickly. Learn more.

  • Our pharmacies participate in the 340B discount drug program, which provides discounted medications for qualifying patients. We also offer other convenient services, such as medication synchronization and mail-order pharmacy services. Learn more.

  • We provide quality, affordable, easily accessible, and evidence-based mental health treatment with empathy, understanding, and compassion. Our mental health experts listen to patients and develop a plan that supports their immediate and long-term needs. For more information about our behavioral health services, click here.

  • Community Care Connections, Addiction Medicine, offers a comprehensive outpatient treatment program for those struggling with substance use disorder. We provide an individualized plan based on individual needs, delivered with compassion, understanding, honesty, and respect. WEEKEND HOURS
Patients at this location can take advantage of extended weekday evening and weekend hours for non-life-threatening conditions.
