Job Objective: 
Perform routine administrative and clinical tasks in order to ensure physicians and health support staff can provide quality medical care to patients. Provide quality care to patients by supporting CCWV quality initiatives through the implementation and execution of specific procedures and programs. May assist nursing staff in the diagnostic testing, examination, education, and treatment of patients.

Responsibilities and Duties:
  • Places patient in examining room and prepares them for examination. May also assist providers in medical examinations as requested. 
  • Obtains patient medical history and records vital signs (height, pulse, temperature, visual acuity, weight, and measures blood pressure, head circumference where indicated for patients). Asks and documents responses to patient health and social history questions during intake process.  
  • Annotates patient charts with exam and treatment results. Contacts patients with laboratory and specialists’ findings and other various tests. Performs accurate and thorough documentation in the patient chart of results and patient outreach.
  • Daily equipment checks and sanitary duties for equipment.
  • Clean and sterilize instruments daily and/or after immediate use using Autoclave system. Performs quality checks on the Autoclave to ensure proper working condition and documents findings of regular performance checks.
  • Keep stock room neat and orderly and restocks individual examination rooms with standard list of supplies.
  • Monitor medication/vaccination inventory and notify Purchasing Coordinator when supplies run low.
  • Counsels patients in office and on phone concerning medication refills, medication questions and refill requests. Answers patient clinical calls and questions and directs them to appropriate individuals if unable to answer inquiries.
  • Other duties as assigned.

Education, Knowledge and Skills:
  • Training in diagnostic and treatment procedures recommended.
  • High school diploma (or G.E.D.) followed by a two-year medical assisting program.

Job Objective:
Perform and coordinate tasks and activities in several areas of the clinic. Provide quality care to patients by supporting CCWV quality initiatives through the implementation and execution of specific procedures and programs.

Responsibilities and Duties:
  • May assist providers in medical examinations as requested. 
  • Obtains and records patient vital signs (height, pulse, temperature, visual acuity, weight, and measures blood pressure, head circumference where indicated for patients).
  • Escort patient to exam room and initiate the patient check-in process. This includes verifying demographic information, emergency contacts, next of kin, preferred pharmacy, HIPAA Notice of Privacy Practices, income level, medical release forms, etc. and documenting in the correct fields within the patient’s electronic health record. 
  • Ensure that all signed paperwork is properly barcoded and then scanned into the patient’s electronic health record with the correct document name.
  • Register the patient for the patient portal, if applicable. Provide education as to the use of the system, highlight what communication can be made via the portal and the access the patient will have to their health information. Provide the patient with instructions on how to utilize the system and generate passcode that allows for initial access.
  • Assists with Pain Management patient pill counts and drug screenings
  • Perform federal urine drug screenings after appropriate training.
  • Collects finger stick A1c’s
  • Perform blood draws based upon training level.
  • Calibrate, and maintain laboratory equipment to ensure efficient performance
  • Annotates patient charts with lab results. Contacts patients with laboratory and specialists’ findings and other various tests. Performs accurate and thorough documentation in the patient chart of results and patient outreach.

Job Objective:
The Licensed Practical Nurse (LPN) is responsible to the School Based Health Program Coordinator for rendering nursing care in assigned area in terms of individualized patient needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Coordinator.

Position Responsibilities/ Essential Duties:
  • Assist medical provider in implementing school based health center program and promote its purpose, function and success.
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient plan of care based on the identified patient problems/needs.
  • Carry out medical provider’s orders accurately, directly and indirectly.
