Job Objective:
This position is responsible for managing an assigned caseload of adolescent and adult patients having long-term mental illness, and/or chemical dependency related diagnoses. The case manager is responsible for assessing needs, developing, implementing and reviewing treatment plans, and coordinating services for individuals and families served. The case manager provides services and assists the mental health professionals in the development and delivery of services to individuals challenged by mental illness and/or addiction.

Position Responsibilities/Essential Duties:
  • Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on all patients entering caseload.
  • Develops appropriate treatment/service plans with patients as the means for implementing appropriate services and developing effective alliances with patients.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the treatment plan in order to help the patient achieve the stated goals and objectives.
  • Provides face-to-face reviews with patients and/or relevant staff on a regular basis, the progress made in achieving treatment goals so that the treatment plan can be modified as necessary to ensure that the goals and objectives are been achieved. The frequency of reviews will be determined by the treating provider and treatment plan.
  • Documents all contacts on a timely basis including face-to-face interviews, meetings with treatment providers, maintains case records in accordance with regulatory standards and requirements.
  • Assists the patients in obtaining appointments with external specialists and setting up patient use of community services.
          a. Refers patients to community resources as needed. Establish and maintain
               relationships with identified service providers.

          b. Ensures that referrals are addressed in a timely manner.
  • Maintains ongoing tracking and appropriate documentation on referral tracking.
           a. Communicates with behavioral health staff any issues or any need of further
               follow up.

           b. Contacts the patient and/or family member regarding any pending referrals, and
                or 
diagnostic testing.
           c. Sends appropriate notifications, reminders and letters to patients

ob Objective:
Brief, problem focused psychotherapy utilizing an integrated care model. The provider provides counseling services including but not limited to assessment, treatment planning, long-term counseling, psycho education, case management, advocacy, and follow-up. Under the direction of the Behavioral Health- Chief Medical Officer, the licensed provider will administer, evaluate and interpret psychological testing; write, monitor and coordinate behavior management plans with the clinical team. The provider will perform treatment therapy in individual and group settings.

Position Responsibilities/Essential Duties:
  • Provide counseling regarding treatment and treatment interfering behaviors. Participate in the Integrated Pain Management/ Behavioral Health Departments for those that have coexisting chronic pain and mental health concerns.
  • Interact with patients to assist them in gaining insight, defining goals, and planning action to achieve effective personal, social, educational, and vocational development and adjustment.
  • Identify psychological, emotional, or behavioral issues and diagnose disorders, using information obtained from interviews, tests, records, and reference materials.
  • Counsel individuals and groups regarding concerns to modify behavior or to improve personal, social, and vocational adjustment.
  • Develop and implement patient treatment plans based on research, clinical experience, and patient histories.
  • Review and evaluate patients’ progress in relation to measurable goals described in treatment and care plans. Continually assess patient needs, service availability and appropriateness of treatment plan. Regularly discuss the treatment of problems with patients.
  • Interview patients, review records, and confer with care teams in order to evaluate patients’ mental and physical condition, and to determine their suitability for participation in a specific behavioral program.

Job Objective:
The Integrated Case Manager will utilize appropriate BH screening tools such as the SBIRT, to evaluate for potential BH needs of chronic pain patients. Implement and evaluate efficacy of BH interventions with chronic pain patients. Develops a care plan in coordination with Pain Management and Behavioral Health providers into the patient’s overall plan of care. Responsible for coordinating counseling services, oversees data implementation, and provides case management services for other patient needs.  Ensures closed-loop follow-up of all referrals for service to ensure prompt and efficient service.  
 
Position Responsibilities/Essential Duties:
  •  Works in coordination with CCWV Pain Management team in developing, adopting, maintaining, communicating, and improving behavioral health practice guidelines of all services provided by the behavioral health program to chronic pain patients.
  • Will serve as the primary connection between patient’s pain management provider and behavioral health provider in order to concerning potential disorder and treatment option availability.
  • Performs internal and external behavioral health referrals to partnering behavioral health agencies that specialize in particular mental and behavioral health disorders.
  • Maintains on-going tracking and appropriate documentation of referral tracking.
    • Communicates with pain management/behavioral health staff any issues or any needs of further follow up. 
    • Contacts the patient regarding pending referrals and/or diagnostic testing. 
    • Sends appropriate notifications, reminders and letters to patients communicating referral status.   Review details and expectations about the referral with patients.
    • Ensure the patients electronic health record is up to date with information on specialist consults including documentation of scheduled appointment, results from specialist and treatment recommendations.
  •  Provides on-going supportive and /or case management functions in accordance with the problems, needs and the strategies within the plan of care in order to help the patients to achieve the stated goals and objectives. 
  • Documents all services on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence, and maintains case records in accordance with the regulatory standards and requirements.
  • Coordinates services with other members of the behavioral health team, updates treatment plans as necessary and schedules assessments, and coordinates for the internal and external referral services based on clinical indicators.
  • Receives, reviews and analyzes patient SBIRT screening results as well as other appropriate screening results, for high risk behaviors in order to properly refer to specialized behavioral health treatment providers.
  • Complete special projects as requested, e.g. surveys, statistical reports, departmental specific reports, patient services tracking information.

Job Objective:
Approved Licensed Professional Supervisor (ALPS) is responsible for ensuring that the medically related emotional and social needs of our adolescent and adult patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines and regulations. The ALPS will provide services designed to meet the specific mental health and/or substance abuse needs of patients as they relate to their diagnosis.
 
Clinical Supervision:
  • Will provide clinical supervision to Provisional Licensed Professional Counselors on staff. As clinical supervisor, will be required to meet with Provisional LPC in face-to-face meetings providing feedback and guidance for portions of clinical practice that may raise questions or concerns as the social worker works in the field, as well as an opportunity to for the social worker to reflect to see how their own biases and gaps in knowledge can impact the quality and direction of their clinical work.

Position Responsibilities/Essential Duties:
  • Engage patients and families in active care planning to focus on behavioral, clinical, social and environmental concerns and needs of patients.
  • Apply counseling theory, knowledge, methods, ethics and the professional use of self in counseling practice including assessment, planning, intervention, counseling, evaluation and case.
  • Conduct clinical or psychotherapeutic services, under the clinical supervision of another experienced and credentialed behavioral health professional, including individual, family and group methodologies.
  • Assesses the patient’s knowledge of their behavioral health condition and provides education and self-management support based on the patient’s preferences.
  • Will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team. Works cooperatively with treatment team members, individuals and the family in provision of services to meet the goals developed in the patient’s treatment care plan.
  • Provide therapeutic interventions to the patient in order to enhance and foster the patient’s optimal functioning and interactions.
  • Continually assess patient needs, service availability and appropriateness of treatment plan.
  • Create and maintain a comprehensive inventory of local community resources, improving accessibility for patients and providers, and linking patients with the appropriate support services. 
  • Provide the collaborative documentation for service delivery including treatment goals, progress notes, and billing and submit said documentation as specified by set standards.

  • Assist school counselors, school social workers and school psychologists with coordinating universal prevention and targeted interventions with students, acting as an intervention specialist.
  • Provide screening to identify at-risk students and provide interventions and referrals, as needed, for students and families.

  • Provide individual, group and family therapy, based on need (trauma, divorce, grief, etc.) maintaining a full caseload, based on mental health standards and ethics.
  • Provide follow-up and implementation of reintegration plans for students who are returning to school from hospitalizations and or facility stays; and comprehensive follow-up to all clients served.
  • Participate in meetings as necessary (Expanded School Mental Health [ESMH] leadership team, school leadership team, staffing, trainings, SAT/IEPs, community forums, etc.)
  • Develop and implement a workforce development training plan to increase the mental health awareness and literacy of school staff, administrators, parents and others who interact with school-aged youth to recognize the signs and symptoms and mental illness and link them to appropriate services.
  • Assist district in preventing student dropout, decreasing discipline referrals and improving attendance.
  • Maintain data on students served in compliance with the ESMH Statement of Work, grant requirements, ethical standards, and the LEA.
  • Be familiar with school/county disciplinary policies and procedures.

Position: Collaborative Care Case Manager (Behavioral Health)

Reports To: Behavioral Health Senior Operations Manager
Location:  Bridgeport/Weston

Job Objective:
Case manager develops a COCM work plan to achieve patient behavioral health goals. Implement and evaluate efficacy of BH interventions with medical patients. Supervise day-to-day operations of the program. Develops strategic plan of action to ensure program deliverables and works with medical providers to fully integrate behavioral health services within the medical model. The case manager is responsible for coordinating psychiatric consultation services,  oversees follow-up care and provides case management for other patient needs. Responsible for implementation of Quality Improvement clinical indicators, staff development, and department representation.
 
Position Responsibilities/Essential Duties:
  • Works in coordination with the CCWV medical team in developing, adopting, maintaining, communicating, and improving behavioral health practice guidelines of all services provided by the behavioral health program.

Works cooperatively with treatment team members, individuals and the family in provision of services to meet the goals developed in the patient’s treatment care plan.
  • Provide psychosocial diagnostic assessment, provide a DSM5 diagnosis and provide treatment recommendations including level of care.
  • Deliver in-office mental health and /or substance abuse intervention services to patients as requested.
  • Provide therapeutic interventions to the patient in order to enhance and foster the patient’s optimal functioning and interactions.
  • Continually assess patient needs, service availability and appropriateness of treatment plan.
  • Provide the collaborative documentation for service delivery including treatment goals, progress notes, and billing and submit said documentation as specified by set standards.
  • Promotes positive inter-department cooperation. Willingly participates in applicable committees or task forces, demonstrates a positive behavior in identifying and resolving complex or out of policy operation problems.
  • Other projects and duties as assigned.

Education/Qualifications/Experience:

Education
  • Bachelor’s degree in social work from an accredited school of social work.

Qualifications
  • Licensed by the State Board of Social Work.
  • Certified Clinical Alcohol, Tobacco, and other Drug Social Worker (C-CATODSW) is highly preferred.
  • Must be able to successfully complete a pre-employment background check and fingerprint analysis.

Experience
  • Two (2) years of supervised social work experience in a health care setting, working directly with individuals.
  • Experience working directly with individuals who have chemical dependencies is highly preferred.
  • Experience with Motivational Interviewing, Self-Management and Patient Engagement strategies.
  • Experience with Medicare and Medicaid population, insurance providers and telephonic engagement.
  • Demonstrated experience in working with an underserved population.

Skills:
  • Must demonstrate excellent interpersonal communication skills, the ability to engage with varying patient populations.
  • Must be flexible and adaptable to change. Demonstrate the ability to work independently, as well as collaborate with multi-disciplinary teams and health care providers. Advocate for patients within medical community settings.

Job Objective:
The licensed graduate social worker (LGSW) is responsible for ensuring that the medically related emotional and social needs of our adolescent and adult patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines and regulations. The LGSW will provide services designed to meet the specific mental health and/or substance abuse needs of patients as they relate to their diagnosis.

Position Responsibilities/Essential Duties:
  • Engage patients and families in active care planning to focus on behavioral, clinical, social and environmental concerns and needs of patients.
  • Apply social work theory, knowledge, methods, ethics and the professional use of self in social work practice including assessment, planning, intervention, counseling, evaluation and case.
  • Conduct clinical or psychotherapeutic services, under the clinical supervision of another experienced and credentialed behavioral health professional, including individual, family and group methodologies.
  • Assesses the patient’s knowledge of their behavioral health condition and provides education and self-management support based on the patient’s preferences.
  • Will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team. Works cooperatively with treatment team members, individuals and the family in provision of services to meet the goals developed in the patient’s treatment care plan.

  • Experience working directly with individuals who have chemical dependencies is highly preferred.
  • Experience working with adolescent populations, their families and socioeconomic conditions.
  • Demonstrated experience in working with an underserved population.
