Job Objective:
The Psychiatrist will utilize teleconference software to interact with patients and colleagues. The Psychiatrist will evaluate medical and psychosocial conditions; develop and implement treatment plans, including prescription of psychotropic medications; monitoring and evaluating treatment results.

Position Responsibilities/Essential Duties:
  • Develop familiarity with Zoom tele-conference software and will tailor approach toward the unique challenges of communicating with patient’s remotely. Will communicate with rest of Behavioral Health division and broader Community Care of WV staff via Zoom.
  • Evaluates patients by interviewing patient, family, and other persons; conducting physical examinations; observing behaviors; reviewing medical history and related documents; selecting, administering, and interpreting psychological tests; ordering laboratory test and evaluating results.
  • Develops treatment plans by determining nature and extent of cognitive, emotional, developmental, social, and behavioral disorders; establishing treatment goals and methodologies.
  • Treats patients by utilizing psychotherapeutic methods and medications; discussing progress towards goals with patients; providing instructions; monitoring effect of medications; supervising staff provision of services.
  • Maintains interdisciplinary treatment by reviewing treatment plans and progress; consulting and collaborating with primary care physicians, mental health therapists, nurses and other health care providers.
  • Improves staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues.
  • Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries.
  • Teach, take continuing education classes, attend conferences or seminars, or conduct research and publish findings to increase understanding of mental, emotional, or behavioral states or disorders.
  • Prepare and submit case reports or summaries to government or mental health agencies.
  • Provide the collaborative documentation for service delivery including treatment goals, progress notes, and billing and submit said documentation as specified by set standards. Maintains complete and accurate records and reports regarding patients’ histories and progress, services provided, and other required information.
  • Promotes positive inter-department cooperation. Willingly participates in applicable committees or task forces, demonstrates a positive behavior in identifying and resolving complex or out of policy operation problems.
  • Telemedicine services will be provided by licensed and credentialed behavioral health providers who are full/part time employees or have a current contract with Community Care of West Virginia, Inc. The clinical care will be provided within the scope of their license.

Job Objective:
The licensed graduate social worker (LGSW) is responsible for ensuring that the medically related emotional and social needs of our adolescent and adult patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines and regulations. The LGSW will provide services designed to meet the specific mental health and/or substance abuse needs of patients as they relate to their diagnosis.

Position Responsibilities/Essential Duties:
  • Engage patients and families in active care planning to focus on behavioral, clinical, social and environmental concerns and needs of patients.
  • Apply social work theory, knowledge, methods, ethics and the professional use of self in social work practice including assessment, planning, intervention, counseling, evaluation and case.
  • Conduct clinical or psychotherapeutic services, under the clinical supervision of another experienced and credentialed behavioral health professional, including individual, family and group methodologies.
  • Assesses the patient’s knowledge of their behavioral health condition and provides education and self-management support based on the patient’s preferences.
  • Will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team.   • Participate in meetings as necessary (Expanded School Mental Health [ESMH] leadership team, school leadership team, staffing, trainings, SAT/IEPs, community forums, etc.)
  • Develop and implement a workforce development training plan to increase the mental health awareness and literacy of school staff, administrators, parents and others who interact with school-aged youth to recognize the signs and symptoms and mental illness and link them to appropriate services.
  • Assist district in preventing student dropout, decreasing discipline referrals and improving attendance.
  • Maintain data on students served in compliance with the ESMH Statement of Work, grant requirements, ethical standards, and the LEA.

Position: Behavioral Health Case Manager

Job Objective:
This position is responsible for managing an assigned caseload of adolescent and adult patients having long-term mental illness, and/or chemical dependency related diagnoses. The case manager is responsible for assessing needs, developing, implementing and reviewing treatment plans, and coordinating services for individuals and families served. The case manager provides services and assists the mental health professionals in the development and delivery of services to individuals challenged by mental illness and/or addiction.

Position Responsibilities/Essential Duties:
  • Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on all patients entering caseload.
  • Develops appropriate treatment/service plans with patients as the means for implementing appropriate services and developing effective alliances with patients.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the treatment plan in order to help the patient achieve the stated goals and objectives.
  • Provides face-to-face reviews with patients and/or relevant staff on a regular basis, the progress made in achieving treatment goals so that the treatment plan can be modified as necessary to ensure that the goals and objectives are been achieved. The frequency of reviews will be determined by the treating provider and treatment plan.
  • Documents all contacts on a timely basis including face-to-face interviews, meetings with treatment providers, maintains case records in accordance with regulatory standards and requirements.
  • Assists the patients in obtaining appointments with external specialists and setting up patient use of community services.
          a. Refers patients to community resources as needed. Establish and maintain relationships                  with identified service providers.
          b. Ensures that referrals are addressed in a timely manner.
  • Maintains ongoing tracking and appropriate documentation on referral tracking.
           a. Communicates with behavioral health staff any issues or any need of further follow up.
           b. Contacts the patient and/or family member regarding any pending referrals, and or       
               diagnostic testing.
           c. Sends appropriate notifications, reminders and letters to patients communicating
​               referral status. Review details and expectations about the referral with patients.
           d. Ensure the patient’s electronic health record is up to date with information on specialist
               consults including documentation of scheduled appointment, results from specialist and
               treatment recommendations.
  • Meets regularly with immediate supervisor as a means of enhancing professional growth, reviewing case management activities and addressing administrative issues.
  • Maintains close communication with the consulting psychiatrist and support staff regarding medication compliance, side effects of medication, and medication changes and alerts these members of the staff to any changes in patient which may suggest the need for intervention.

Position Title: Psychiatric Nurse Practitioner/Physician Assistant  
Position Type: Full-time 
Reports To: Director of Behavioral Health
Location: Multiple Locations

Job Description:
The Physician Assistant/Nurse Practitioner performs direct psychiatric care in the medical office setting under the supervision of a licensed psychiatrist. The Physician Assistant/ Nurse Practitioner obtains medical histories and performs medical evaluations on clinic patients, treats psychiatric problems, maintains records, and provides health education. 

Medical Responsibilities and Duties: 
  • Focus primarily on the population identified or suspected behavioral/mental health conditions and establishes therapeutic relationships with these patients to add psychiatric expertise to the medical management plan and mitigate any negative impact that behavioral conditions can have on patient overall health and well-being. 
  • Provide all aspects of patient psychiatric healthcare, including diagnosis, and treatment of a variety of psychiatric and common medical conditions. 
  • Diagnosis and support the management of comorbidities and complications common to patients with behavioral/mental health conditions. 
  • Provide direct psychiatric services through initial comprehensive evaluation, diagnosis, and treatment of patients, including prescribing and administering medications. 
  • Prescribe and monitor behavioral health medications prescribed. 
  • Handle psychiatric emergencies as needed.  
  • Communicate with the patient’s primary care provider regarding changes in conditions or potential medication changes. 
  • Collaborate with supervising physician in regards to particularly complex behavioral/mental conditions. Maintain partnership with supervising physician through regular communication, care plan development, medication management, etc. 
  • Participate in quality assurance, educational, and professional committees as assigned by the Chief Medical Officer. 
  • Actively participate in regular multi-disciplinary team meetings to assure behavioral health issues considered and addressed for all high risk patients. 
  • Other duties, as assigned.

Additional Responsibilities: 
He/she participates in other assigned duties as designated by the Chief Medical Officer.  Also perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances. 

Education, Knowledge, Skills and Abilities: 
  • Ability to establish and maintain positive, caring relationships with executive management, managers, physicians, non-physician providers, support staff, other departments, and patients/families. 
  • Advanced Cardiac Life Support (ACLS) certified. 
  • Ability to work productively and effectively within a complex environment, handle multiple/challenging priorities. 
  • Critical thinking, analytical and problem solving abilities required as related to various aspects of patient care. 
  • Good clinical judgement
  • Critical thinking skills necessary to exercise and to lead others in application of acceptable standards of practice. 
  • Awareness of NCQA requirements and CMS guidelines is helpful. 

Physician Assistant:
  • Successful completion of an approved physician assistant program. Currently approved by The National Commission on Certification of Physician Assistants. Must be currently licensed as physician assistant in the state. 
  • Psychiatric certification for Physician Assistant by appropriate national organization is highly preferred. However, if not certified, CCWV will provide intensive training under the supervision of a licensed psychiatrist,  in a new model of team-based, integrated care in which behavioral health clinicians, primary care providers, and pharmacists all work closely together, sharing the same electronic health record. 
  • Current Federal Drug Enforcement Administration (DEA) number 

Nurse Practitioner:
  • National certification as a Nurse Practitioner
  • Current registered nurse license and an NP license to practice in the State of West Virginia
  • Current prescriptive authority from the State Board of Nursing
  • Current Federal Drug Enforcement Administration (DEA) number 
  • Certified Registered Psychiatric Nurse Practitioner is highly preferred. Job Objective:
The Psychiatrist will evaluate medical and psychosocial conditions; develop and implement treatment plans, including prescription of psychotropic medications; monitoring and evaluating treatment results. 

Position Responsibilities/Essential Duties: 
  • Evaluates patients by interviewing patient, family, and other persons; conducting physical examinations; observing behaviors; reviewing medical history and related documents; selecting, administering, and interpreting psychological tests; ordering laboratory test and evaluating results. 
  • Develops treatment plans by determining nature and extent of cognitive, emotional, developmental, social, and behavioral disorders; establishing treatment goals and methodologies. 
  • Treats patients by utilizing psychotherapeutic methods and medications; discussing progress towards goals with patients; providing instructions; monitoring effect of medications; supervising staff provision of services. 
  • Maintains interdisciplinary treatment by reviewing treatment plans and progress; consulting and collaborating with primary care physicians, mental health therapists, nurses and other health care providers. 
  • Improves staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues. 
  • Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries. 
  • Teach, take continuing education classes, attend conferences or seminars, or conduct research and publish findings to increase understanding of mental, emotional, or behavioral states or disorders. 
  • Prepare and submit case reports or summaries to government or mental health agencies.
  • Provide the collaborative documentation for service delivery including treatment goals, progress notes, and billing and submit said documentation as specified by set standards. Maintains complete and accurate records and reports regarding patients’ histories and progress, services provided, and other required information. 
  • Promotes positive inter-department cooperation. Willingly participates in applicable committees or task forces, demonstrates a positive behavior in identifying and resolving complex or out of policy operation problems. 
  • Other projects and duties as assigned.

Education/Qualifications/Experience:
Education  
  • Medical degree from an accredited school and licensed to practice in West Virginia. 

Qualifications 
  • Board certification or board eligibility in psychiatry.  
  • DEA certificate and WV Controlled Substances registration. 
  • 3. Must be able to successfully complete a pre-employment drug screening and fingerprint national background check. 

Experience
  • 1.  Two (2) years of practical therapy experience. 
  • 2. Experience working directly with individuals who have chemical dependencies is highly preferred. 
  • Experience with Motivational Interviewing, Self-Management and Patient Engagement strategies. 
  • Experience with Medicare and Medicaid population, insurance providers and telephonic engagement. 
  • Demonstrated experience in working with an under-served population.

Job Description:
The psychologist provides substance abuse counseling services including but not limited to assessment, treatment planning, long-term counseling, psycho education, case management, advocacy, and follow-up. Under the direction of the Chief Medical Officer, the psychologist will administer, evaluate and interpret psychological testing; write, monitor and coordinate behavior management plans with the clinical team.

Responsibilities and Duties:
  • Counsel patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
  • Conduct chemical dependency program orientation sessions with patients.
  • Develop patient treatment plans based on research, clinical experience, and patient histories.
  • Review and evaluate patients’ progress in relation to measurable goals described in treatment and care plans.
  • Interview patients, review records, and confer with care teams in order to evaluate patients’ mental and physical condition, and to determine their suitability for participation in a specific behavioral program.
  • Continually assess patient needs, service availability and appropriateness of treatment plan.
  • Plan and implement follow-up and aftercare programs for patients recently discharged from treatment programs.
  • Provide patient or family members with information about addiction issues and about available services and programs, making appropriate referrals when necessary. Counsel family members to assist them in understanding, dealing with, and supporting patients.

  • Specialty certification in addiction or substance abuse therapy is highly preferred.
  • Must be able to successfully complete a pre-employment background check and fingerprint analysis.

  • Demonstrated ability in group therapy, group management, individual therapy, and family counseling.

Job Description:
Provides psychosocial assessment of the individual and family, including limitations, deficits and strengths in their current situation and environment, as well as functional level. May conduct a psychiatric social work assessment according to the Diagnostic and Statistical Manual of Mental Disorders. Develops treatment plans with appropriate and competent interventions. The LICSW will provide services designed to meet the specific mental health and substance abuse needs of patients as they relate to their diagnosis.

Position Responsibilities/Essential Duties:
  • Engage patients and care givers telephonically and in person in active care planning to focus on behavioral, clinical, social and environmental concerns and needs of patients.
  • Create and maintain a comprehensive inventory of local community resources, improving accessibility for patients and providers, and linking patients with the appropriate support services.   
  • Assesses the patient’s knowledge of their medical condition and provides education and self-management support based on the patient’s preferences.
  • Will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team. Works cooperatively with treatment team members, individuals and the family in provision of services to meet the goals developed in the patient’s treatment care plan.
  • Provide psychosocial diagnostic assessment, provide a diagnosis and provide treatment recommendations including level of care.
  • Deliver in-office mental health and /or substance abuse intervention services to patients as requested.
  • Provide therapeutic interventions to the patient in order to enhance and foster the patient’s optimal functioning and interactions.
  • Continually assess patient needs, service availability and appropriateness of treatment plan.
  • Provide the collaborative documentation for service delivery including treatment goals, progress notes, and billing and submit said documentation as specified by set standards.
  • Promotes positive inter-department cooperation. Willingly participates in applicable committees or task forces, demonstrates a positive behavior in identifying and resolving complex or out of policy operation problems.

  • Experience working directly with individuals who have chemical dependencies is highly preferred.

Clinical Supervision:
May be required to provide clinical supervision to Licensed Graduate Social Worker(s) (LGSW) and Licensed Certified Social Workers (LCSW) on staff. As clinical supervisor, will be required to meet with social worker in face-to-face meetings providing feedback and guidance for portions of clinical practice that may raise questions or concerns as the social worker works in the field, as well as an opportunity to for the social worker to reflect to see how their own biases and gaps in knowledge can impact the quality and direction of their clinical work.

  • Apply social work theory, knowledge, methods, ethics and the professional use of self in social work practice including assessment, planning, intervention, counseling, evaluation and case.
