The Patient-Centered Medical Home (PCMH) recognition from the NCQA means our health centers provide high-quality, patient-focused care. This includes better access to doctors, clear communication, and support for your medical, behavioral health, and social needs. All eligible Community Care locations have earned this recognition, which is renewed yearly to meet high standards. 

These improvements help us offer more services, like two-way texting, the Patient Portal, extended CareXpress hours, online scheduling, digital check-in, after-hours provider access, and care coordination for patients with chronic conditions.

Compliance

As a Federally Qualified Health Center (FQHC), Community Care’s operations and finances are carefully reviewed and regulated by a branch of the federal government entitled Health Resources & Services Administration (HRSA) and the West Virigina Department of Health (WVDOH)

As an FQHC and HRSA entity, CCWV falls under the Federal Tort Claims Act (FTCA), which assists with the cost of insurance coverage and allows us to invest in high-quality primary health care services.  

 CCWV also adheres to the Privacy and Security Standards for the protection of individually identifiable health information outlined in the Health Insurance Portability and Accountability Act (HIPAA). Posted at all locations are Community Care’s Patient Rights and Responsibilities and Privacy Practices, which details our commitment to privacy and HIPAA standards.

FQHC's are community-based organizations that provide comprehensive primary and preventive care, including health, oral, and mental health/substance abuse services to persons of all ages, regardless of their ability to pay or health insurance status. Their mission is to enhance primary care services in under-served communities. CCWV is a Federally Qualified Health Center. Each of its sites is a deemed facility under the Federal Tort Claims Act and is covered by 42 U.S.C. 233. CCWV health centers receive HHS funding and have Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
